Thank you for your interested in signing up as a Volunteer for the 4th Annual Bark Back Benefit. Volunteers must be at least 17 years of age or older to volunteer.

The Bark Back Benefit is an annual music festival hosted by Spred the Dub that benefits Peggy Adams Animal Rescue League. Please come to the event ready to work hard, have fun, and raise money to benefit pets in need in Palm Beach County.

I give permission to The Peggy Adams Animal Rescue League to verify any information given.
I agree to support Peggy Adams Animal Rescue League's mission and decisions, and always represent
them in a positive manner. I verify I am over the age of 17 and understand Peggy Adams Animal
Rescue League cannot give hours towards court appointed community service at this event.

I, BARK BACK BENEFIT VOLUNTEER, hereby release the Peggy
Adams Animal Rescue League from any and all responsibility and liability of any nature whatsoever in
connection with any and all activities which I may participate in on behalf of or in any way associated
with the League, and any and all injuries of any nature whatsoever which may arise out of, result
from, or be associated with said activities. I understand that all records, information, cruelty cases and
ongoing investigations are to be kept strictly confidential. I further understand that records, information
pertaining to clients, donors, staff and other volunteers is confidential in nature. Furthermore, I will
comply with all regulations, policies and procedures of the League, and understand that my failure to
do so will result in my termination as a volunteer. I hereby, execute this release freely and voluntarily
knowing the same is a legal document, and recognizing that I had the opportunity to seek legal
counsel pertaining to this document.

I, intending to legally bind myself, administrators, personal representatives or assignees or any party
holding a durable power of attorney for my actions, do hereby release the Peggy Adams Animal
Rescue League, its employees, Board of Directors, volunteers, or any other individuals associated with
the Peggy Adam Animal Rescue League from responsibility for any injury or damages to either myself
or my pet as part of my participation in this event or any related activities. I grant permission to any
and all of the foregoing to use any photograph, motion pictures, videotapes, or any other record
from the event for any lawful purpose. If the participant is under 18 years of age at the time of
registration, the participant's parent or legal guardian must completely review this Waiver and
Release. The parent or legal guardian understands and consents to its terms, and authorizes the
participation of the registrant by his/her acceptance below.
